The Molkadh Mountains are a small part of the Boethiah's Spine region. Although the borders are not perfectly clear, the area starts in the southwest at Ranyon-ruhn and seems to encompass all of the mountainous area north of the road to Llothanis and west of the coastal road to Tel Ouada. The Molkadh are one of the oldest settled parts of Boethiah's Spine, however most people now prefer to live in more comfortable places, leaving only Ranyon-ruhn, Sarvanni and Ashamul still officially inhabited. The mountains are also home to the terrifying canyon known as Aranyon Pass.
The sub-region is ruled from Ranyon-ruhn, making its current governor Master Mithras.

- Ashamul — A tiny troglodytic community in the Boethiah's Spine.
- Ranyon-ruhn — A small Telvanni settlement with an ebony mine.
- Sarvanni — A small molecrab farm in the Boethiah's Spine.

- Boethian Mountains Lighthouse — A beacon of light on the western shore of the Sea of Ghosts, east of Sarvanni.
- Dunada-Nammu — A burial chamber north of Ranyon-ruhn inhabited by undead.
- Erushara Ancestral Tomb — A medium-sized tomb halfway along Aranyon Pass due south of Sarvanni, inhabited by undead.
- Midannecne — A small cave just south of the Boethian Mountains Lighthouse, occupied by undead and a wanted necromancer.
- Shasharanit — A medium-sized cave in Sunad Mora containing animals.
- Verdunasha-Beracavada Egg Mine — An abandoned egg mine, southwest of Sarvanni, that has been taken over by bandits.
